Namibia is a country in Southern Africa.  Its western border is the Atlantic Ocean. It shares land borders with Angola and Zambia, to the north, Botswana to the east and South Africa to the east and south. Although it does not border Zimbabwe, less than 600 feet of the Botswana right bank of the Zambezi River separates the two countries. Its capital and its largest city is Windhoek.  Its population is just over 3 million people who are sparsely located--making it one of the most sparsely populated countries in the world.
This country has been inhabited since prehistoric times by the Khoi, San, Damara, and Nama people. Around the 14th century immigrating Bantu people arrived as part of the Bantu expansion.  From the 1600 Ovambo formed kingdoms such as Ondonga and Oukwanyama.
In 1884 the German Empire established rule over most of the territory, forming a colony known as German West Africa. Between 1904 and 1908, German troops waged a punitive campaign against the Herero and Nama. German rule ended during the first World War with a 1915 defeat by South African forces. After the end of the war by 1920 , the League of Nations mandated administration of the colony to South Africa.
Today Namibia is a stable parliamentary democracy. It is also a unitary semi presidential republic. There is a president, vice president, prime minister and his deputy, The chief justice heads the legal side while in the legislature there is an upper house and a lower house.
Blacks make up the main ethnic group where 87% of this group lives there. Colored account for 6.5%, Whites just 6% and Others 0.5%. The main religion is Christianity, with 87.9% of the population identifying with this religion. Just 10.2% are observers of traditional faiths, and 1.6% have no religion.
The economy is driven by agriculture, mining --mainly,  diamond, uranium, gold, silver and base metals. However, the manufacturing sector is relatively small. Namibia's current GDP stands at $30.6 billion USD.

Some Facts About Ovarian Cancer Recurrence
Picture
Ovarian cancer is usually detected in the late stages. It is often widespread (metastasized) when diagnosed.
Your doctor will remove as much of the tumor as possible with surgery. You will need chemotherapy. When your body has no tumor left, you are in remission.
Ovarian cancer relapses often. 23% of people have a relapse within 6 months of stopping chemotherapy and another 60% after 6 months.

Picture
Your physician will send the tumor removed during surgery to the laboratory.
Recurrence risk depends on the type of ovarian cancer.
High-grade serous adenocarcinomas and endometrioid carcinomas are the most likely to recur. Other types like clear cell adenocarcinoma , mucinous adenocarcinoma, low-grade endometrioid adenocarcinoma, and others have a lower risk.
Women with stage 1 ovarian cancer have a 5-year survival of 92%. Stage 3 and 4 ovarian cancers relapse early and often.

Picture
Like other cancers, ovarian cancer can come back in three different locations.
*Local Recurrence. The cancer returns at the ovary's  location, even if the ovary was removed.

* Regional Recurrence. The cancer reappears in the lymph nodes near the ovaries.

* Distant Recurrence.
The cancer appears in distant organs like the bones, liver, lungs, or brain.
More than 80% of women have recurrence in multiple places.

Ovarian cancer can return after hysterectomy.
Ovarian cancer is treated by extensive surgery. The uterus, fallopian tubes, lymph nodes, and other tumor-affected parts, are removed.
Your physician will them prescribe chemotherapy.
But the cancer usually survives because it has already spread when diagnosed. Even with such massive surgery, ovarian cancer does recur.

Picture
Frequent checkups are vital for catching recurrence early. In the first two years your doctor will see you every 3 months.  Later, the visits can be every 4 to 6 months.
Your doctor will decide depending on your earlier disease and physical examination, if testing is needed. This test could include vaginal cytology,  measurement of tumor markers, computed tomography (CT), magnetic resonance imaging (MRI), and positioning emission tomography (PET) scans.

You shouldn't depend on looking for symptoms. Your physician won't do that, either.
Ovarian cancer has a very high risk of recurrence. Your physician will screen you for recurrence at regular intervals.  Almost all women with relapse  are without symptoms when detected.
Common symptoms of ovarian cancer recurrence are abdominal bloating or pain, nausea, poor appetite, or weight loss.

Picture
A rise of certain chemicals in your blood tells your doctor about ovarian recurrence.
These chemicals are called tumor markers. Different types of ovarian cancers have different markers.
Cancer antigen-125 (CA-125) is a tumor marker to test for epithelial ovarian cancer. Other useful markers are CA 19-19, HE-4 , and CEA.
Alpha fetoprotein (AFP) human chorionic gonadotropin (HCG) and hormones like estrogen inhibin, and testosterone are markers for other types.

The CA-125 is not a magic marker.
A raised CA-125 doesn't always mean that your cancer is back. You can have high CA-125 without cancer recurrence. Ovarian cancer can return without high CA-125 levels.
Raised CA-125 tells your doctor to look out for recurrence. Starting chemotherapy with only raised CA-125 levels doesn't provide any benefit. It also reduces quality of life.
This tumor marker helps asses the response to treatment, if it was raised before treatment.

On Halloween in 1955, Marilyn Damman took her two-year old- son Steven and seven-month-old daughter, Pamela to a supermarket in East Meadow, New York. While she went shopping Marilyn let Steven wait outside with his sister, who was inside a carriage. Ten minutes later Marilyn exited the store and was shocked to discover that both Steven and the carriage were gone. Shortly thereafter, the carriage was discovered, about a block and a half away.  However, even though Pamela had been left behind, inside the carriage, Steven was nowhere to be found.
Over the years DNA testing has been utilized to determine if Steven was ever given a new identity. In 2009, a Michigan man, named John Barnes came forward believing he might be Steven, but DNA testing ruled this out. It possible that  an adult, Steven Damman might be living somewhere under a different identity, unaware that he was once taken from his real family.

Chris Jenkins was a 21 year-old student of the university in Minnesota, who visited a Minneapolis bar on Halloween night in 2002. After leaving the bar around midnight, Chris vanished without a trace. He remained a missing person for four months until his body was discovered in the Mississippi river. Since Chris was still wearing his Halloween costume, all indications were that he died shortly after he disappeared. Chris was heavily intoxicated that night and since his cause of death was determined to be drowning, authorities initially believed it was an accident or suicide. His parents refused to believe this and pressed for a more thorough investigation. Finally, in 2006 his death was reclassified as an homicide.

SHORT STORY
Ominous clouds seemed to be holding their breath from the eastern sky, ready to release their fury. Howling winds roared with ferocity as if they were on a mission to wipe out everything below. Some trees were acrobatic as their gymnastic displays would rival most contortionists. Others were not so skilful as their performances were below par to the viewer. They fell on the stage with a thud resulting in torn costumes. Some even exposed their bottoms.  Nonetheless, the orchestra played the symphony, often hitting the crescendo. Thirty-five year-old, Susan was alone and had been asleep in an abandoned cottage that owned no proper windows. She sprang up from the floor where she normally made her bed.
The spacious land was the caretaker of many tall trees, crawling creatures, and phantom visitors who always seem to appear only at nights. Some wore decorated teeth, others, had two or three that blended with their gums. Yet, there were those who were just gummy. Their eyes always seemed to glistened in the dark. Most seemed to go to the same hairdresser who had a bias for silvery thin hair.
These visitors always seemed happy. Some laughed with gusto while others would let go a shrieking sound. And they often danced to a rhythm that was distant. Susan detested their visits. But they loved her company. Funnily, they never touched her.
That night the clouds finally broke and collapsed across the land. Barrels of water poured from heaven. Susan, though huddled in a corner felt strangely safe. She was conscious that the rains came from a good place-- the heavens. So, she reasoned she would be unbothered by her nightly visitors.
Through the torrential downpour and unrelenting assault of the winds a familiar song was heard. Susan's friends came out and took the stage. Their costumes were varied. Most sported ragged apparels and ratty shoes. But they laughed explosively. The cottage shook. Susan quivered. The music became louder. And the troupe danced and danced and danced. Three members of the cast broke ranks and got closer to Susan. She screamed. Their faces seemed to touch hers. She ran in horror.
Her parents heard her scream and found her fallen off her bed. It was Halloween morning.    © Wallace Paul October, 2024.
